Although face to face consultations are now being screened.
This is important for the health and safety of you, vulnerable patients and staff.
It also minimises becoming an exposure site, which would result in hundreds of patient appointments being cancelled.
CONFIRMING YOUR APPOINTMENT
Please read the following, then if appropriate, please reply “All clear” to the SMS you received from Dr Semple’s rooms.
1- In the last 14 days have you?

- Visited any exposure sites (see below), even if you have received a negative test
- Visited Melbourne or NSW for ANY REASON.
2- In the last 14 days have you had any of the following?

- Fever or chills
- Cough
- Sore throat
- Runny nose
- Headache
- Loss of sense of smell or taste
3- In the last 14 days have you had contact with ANYONE ELSE that would answer YES to any of the above?
If you are not able to answer NO to ALL the above questions, or you wish to reschedule your appointment please contact the rooms on 52224915, 0458 636 149 or email admin@drsemple.com.au

ON THE DAY OF THE APPOINTMENT
To minimse your exposure and that of others in the waiting room, please remain in your car, or in the downstairs foyer.
We will phone your mobile when it is appropriate for you to attend consulting suite.
